How good are max settings?

How big of a difference would there be in a game that has been maxout to the highest possible settings and the same game on it's lowest settings? Would I be able to notice in gameplay and will it effect my gameplay performance?

Depends on the game.
Settings do have a very large impact on performance, so you do need to tune it to your own personal setup and desires.
Here is one example in SC2. Low is much lower end than medium, from there on its mostly small changes:

It does vary game to game.
Another example is Fo4. I cant run it on anything but low, and it still looks great, but I know it wold look even better (longer render distances, shadows, lighting) on ultra.
