Is my hard-drive causing poor performance?

I would like to specify that I am not a computer expert, I don't know much about computers. I built my current PC back in early 2015, for most of the time that I've had it there have been constant issues with stuttering, freezing, and loading.

I have an EVGA GeForce GTX 960 2GB SuperSC ACX 2.0+ Video Card, and an AMD FX-6300 3.5GHz 6-Core Processor. From what I've read online, I shouldn't have bottleneck problems. I've been trying to play Doom, the game generally runs fine, 60-80 FPS on medium settings, but the game drops down to 1 FPS every now and then, I also have problems with loading and music. Sometimes textures appear blurry and "undetailed", not sure how to describe it, and the music doesn't play. I have this problem on low, medium, and high; graphical settings to do not affect this "stuttering" (not sure if this is the proper term). I have had this happen in many games that I've played.

According to the MSI Burner in-game overlay, my CPU is generally running at around 50%, with my graphics card running at 99%. Everything runs fine, but at times the GPU drops down to around 50% usage, for a brief moment, before going back up to 99%. Based on my task manager, my CPU is always running below 100%, as well as my memory, but my disk is always running at 100%. I'm not sure how to explain what's happening here, the only theory i've come up with is that it may be my hard-drive that is responsible for my poor performance. I've used my current hard-drive for almost 8 years, it's the one part that was never upgraded from my old computer. I've noticed in a lot of my games I have constant loading problems, with textures and models not appearing properly, sometimes the game freezes, and I need to wait a few seconds to continue playing. In battlefield 4, for example, I may find myself running around waiting for the environments to load.

I am uncertain if the hard-drive is, in fact, the problem. If it is, I would like to replace it. I just need to make sure that this is the problem, as I do not want to spend money and have these issues continue. If it is not the hard-drive, I would like to know what may be causing these problems. Any help is appreciated.

Well, I would say that 8 years is quite some time. You really do need to back up the data stored on the drive, until you sort this out.

It would be nice to provide a screen of the SMART report.

Is your HDD running at 100% when not in game? What is your OS?

My HDD doesn't run at 100% when outside of games. It generally runs at 0-5% with low usage. However, when I begin to open/close programs, look through the explorer, switch tabs, etc, it spikes up, sometimes to 100%. The computer becomes slower during this time. I forgot to mention, but my computer also runs very slowly after playing a video game. It speeds up after a few minutes though.

My OS is Windows 10 and my system is x64-based.

The Caution means take no chances, backup all your data, and replace the drive.
You may be able to clone it to make it easier but that isn't guaranteed since the drive is failing.

I had exactly the same scenario and it baffled me until I ran CrystalDiskInfo and discovered the Caution warning. Replaced the drive and no more issues.
 
I totally agree with the other guys. The drive has 77 bad sectors, that were reallocated. They would continue to appear, until the drive fails. You will need to get another one.
As for this one, you can't really know when it is going to fail. It could be in a day, or in several months. So, once you install your new drive and transfer all the data, you can reformat this one and use if for non important data.
