[SOLVED] Dialog Pops Up Every Time On Restart

Hey guys, the dialog pictured below pops up every time I restart my laptop. I have no idea what's causing it. Anyone have any ideas? Thanks.

Check in your startup items:
1.System Preferences>Users & Groups.
2.Select your User ID.
3.Select Login Items.

See what is in there, might be a clue.
