Mystery Graphics Card, need driver, can't find! Assistance Please!

Once upon a time, years ago, I built a PC from scratch. Fast forward to today, and I had to reformat that computer, reinstalling Windows 7 fresh. Now my PC doesn't even acknowledge my graphics card exists. It's using the generic VPN of the motherboard, and because the card doesn't show up in the Device Manager list, I can't find the name in order to search for drivers online.

I have tried examining the card itself, but it only has several stickers with numbers, and no actual identifying name. The only clue I have is that the single attached fan has a 'PowerColor' sticker on it. The PowerColor website, however, lists model numbers, and that's another piece of data I don't have.

My plea: Is there a way I don't know of, that I can use to find the name/model/ID/etc of this card, that can allow me to find the drivers, and fix my problem?
