Is my 4+ year old graphic card going bad?

To describe it as best as I can and it happened twice this week. I built my computer a little over 4 years ago, maybe 5 and a few days ago my computer went to sleep one time and I would wake it up and I get this gray/white static image like you would see on a television. It didn't happen again until today when I'm just playing a Facebook game and I get the gray/white image. I'm thinking either monitor or graphic card.

I upgraded to Windows 10 in the summer and my rig isn't for hardcore gaming, I'm done with that. Just moderate gaming, I do a lot of movie watching and internet surfing. My computer setup is:

AMD Radeon HD 5670
Processor - Intel Core i5-2500k 3.30 GHz
4 GB RAM

I was leaning on that too because ever since I've upgraded to Windows 10, half of the time my computer goes to sleep and I wake it up and the monitor fails to turn on and I would have to reset it. There were times when I had to reset 5 times and I also had to unplug the monitor and plug it back in for it to work. I don't know if it's the Windows 10 thing or one of the other components. I need to be sure.
