Prime95 BSOD within 1 minute?

So I've been having PC issues for a while, here's how it would happen (in chronological order)
1. Every now and then, after fresh boot up the screen would turn a solid color, forcing a reboot (1x)
2. Every first boot of the day after a fresh boot up the screen would turn a solid color, forcing a reboot (1x)
3. Every first boot of the day and every time I close a game application, the screen would turn a solid color, forcing a reboot (2x)
4. 2 first boots a day the screen would turn a solid color, and every time I close a game application, the screen would turn a solid color, forcing a reboot (3x)
5 (PRESENT/NOW) 2 first boots a day the screen would turn a solid color, and can't even play games without my frames randomly dropping to 2-3FPS spikes and then sound glitching out, screen eventually turning a solid color forcing a reboot (4x+)

It's gotten past annoying to literally unplayable, so here's what I've done:

- Installed Windows Updates
- NVIDIA Drivers
- RMA'd my graphics card, so GPU shouldn't be an issue - getting a new one anyway
- Tested the following componets:

1. (POWER SUPPLY) BIOS 12v+ rail: GOOD
2. (MEMORY) Windows Memory Diagnostic: GOOD
3. (HARD DISK) Disk Mark: GOOD
4. (PROCESSOR) Prime95: BAD - check below for diagnostic results!

Running prime95, my PC crashed to a BSOD within a minute. Here's the log: "A clock interrupt was not received on a secondary processor within the allocated time interval. Technical information: *** STOP: 0x00000101 (0x0000000000000031, 0x0000000000000000, 0xFFFFF880009EF180, 0x0000000000000001)"

New parts I'm purchasing (some irrelevant)
- 250GB Samsung 850 EVO SSD
- 8GB (2x4GB) G.Skill Sniper RAM
- EVGA GeForce GTX 720 2GB /w ACX Cooler (Not purchasing, being RMA'd)

My specifications:
Processor: Intel i5-4670k OC'd to 4.1GHz
CPU Cooler: Cooler Master Hyper TX3
RAM: 8GB (2x4GB) G.Skill Sniper 1600Mhz
Motherboard: ASRock Z87M Extreme4
Power Supply: EVGA SuperNOVA NEX 750B (750W 80+ Bronze Certified)
Operating System: Windows 7 Ultimate 64-bit

I'd recommend removing the over clock on the processor and then try running prime 95. A lot of the time if you don't up the voltage in your motherboard or your motherboard cannot output the voltage that you want then it will blue screen, so try removing the overclock and then get back to me.

Well I just used the default "Optimize Turbo Overclock" provided by my BIOS and went from 4.1ghz to 4.0ghz and then used the recommended/suggested voltage and other options.

no crash as soon as i went to desktop, i'll keep you posted.

after a few days of things going alright with the 4.1 turbo, i started to get the same in-game issues of massive frame drops to sub <5fps

Decided to just bring everything back down to stock 3.4 and see how things go, will keep you posted

still having the same issues on all the stock speeds

Default 3.4ghz with 4.0ghz turbo
4.0ghz optimized OC
4.1ghz optimized OC

all of them i still get the blank screen sound glitches in games, but not as frequent.
