Hey guys
So get to the point
I have HIS HD 7790 from last 2.5 years, and from last 2 weeks I am facing Unstable gpu Temps,
At idle the Temps are normal, 35 to 39c, but when I start gaming the Temps bump up to the 100c and game crashes, any idea what is happening
Soon I am upgrading to rx 480 4gb, but till then I don't wanna stop gaming :)

Specs are
CPU: i5 3570
Ram: Kingston 2x4 8gb @ 1.6 ghz
Mobo: Asus p8h61 ml e
HDD: 500gb
Psu: Antec tp650w

Solution
My close friend has an R9 290 and it's about as old as your card, it had a few owners. Even though it's clean and visually seems to be perfect, it slowly dies, starts crashing games when the load is high, the temperatures turn the PC into a cooking pot and it generally behaves weird.
Don't worry, your card is probably at the stage of dying. You can do what MrKrako said, clean it.
Although if cleaning the card does not help, you will have to buy a new one.
You could also chech if it's working fine on someone else's computer.
