Help to troubleshoot Frequent Freeze

Specs:
Cpu: AMD Phenom II X4 955
MotherBoard: MSI Global 870A-G54
Graphics Card: Radeon 6950
HDD: WD 1TB Black
Ram: Corsair Vengeance 8GB ( checked with Cpu-z dram frequency 666 Mhz at 1.5v ) (DDRAM speed is 1600 but mobo/cpu support up to 1333 Mhz)
Power Supply: Silverstone 500w
OS : Windows 10 Fully Updated
My computer would freeze and the image displayed on monitor would be stuck there. Mouse, keyboard would no respond and the computer would not unlock itself and would require a reboot. This would happen randomly. Example it could freeze when using Google Chrome, playing a video on vlc player etc. If a video was being played, loud buzzing sound will be emitted from the speakers. Even when a video is played from flash drive and the computer freezes, I unplugged the flash drive but the stuck video frame is still displayed on the monitor.
When computer is frozen, HDD Led would be solid blue (no activity).
I would also not get a BSOD even if I leave the computer frozen. Thus no dump report and tried with Whocrashed. Also Event Viewer doesn't seem to have anything. (unless I am looking at the wrong places).
Things I have tried to Identify root cause of problem: (currently)
1) I have tried updating GPU drivers to AMD Beta Crimson drivers.
2) Ran memtest86+ from Flash drive. Did 3+ passes over 6.5 Hrs with 0 problems detected.
3) ran HD Tune Bad sector scan with no bad sectors.

4) Reseated Ram, GPU and cleaned contacts with rubbing alcohol. Also used Datavac electric duster to clean out for dust.
(CPU has no integrated graphics card so I am unable to switch)
This problem has been occurring since July 2016 however somehow for around 3 + months I did not get any freezes until about 4 days ago , 28 Nov 2016. There has been no hardware or software changes since then. Computer has been running mostly daily for about 8+h with temps for GPU CPU MOBO in Mid 40 degrees Celsius.

Previously ran these tests also:
Prime95 for cpu. Blend test etc. No crash (it was ran for less than an hour)
Furmark for Gpu. No crash, stable. (max temp was around 67 degrees Celsius)
overall should not be heat related.

Any opinion and input will be greatly appreciated. Thank You.

I have ran Prime95 Blend and Small FFTs. Around 4-5 h mark for both test, Prime95 will have an error. "prime 95 has stopped working". I was using (prime95 v28.9 Build 2) for the test.

My CPU clocks are stock and so are my Ram. What should I do?
