Games cause crash to black screen, audio continues

Hi, hope you can help.

Home built PC has functioned perfectly for a year, but within the last two days started crashing immediately to a black screen when placed under load, often with the audio continuing. This requires a hard reset. This started occurring following a driver update, but I've rolled those back and the issue persists. A fresh Windows install also hasn't solved the issue. The CPU/GPU temps remain normal throughout.

I did however notice the motherboard reporting it had detected a power surge; and within the BIOS it reports the 12v output as always below 11.5v; occasionally switching between 10.3v-10.8v as I watch. Am I correct in assuming this requires an RMA of the PSU? It's a year old EVGA 650 G2, so it was one of the components I'd been led to believe would last the longest.
