Hyper Transport sync flood error

I have been running on my pc i have bought new graphic card HD 6850,it was running perfectly so one day when i format my pc,my pc can't boot.After the motherboard logo appears error Hyper Transport sync flood error occurred on last boot.Prees f1 to resume.I go resume and after windows loading logo my screen went black and restarts.Always do that,can't even boot in safe mode.So i take out my graphic card and let it rest about 1 mounth.After that i was using onboard and before i put it i unnistall all graphic drivers and put my graphic card.It boot it to windows i install the drivers,and when i installed the error again but when i press resume my pc start and i can play games etc...Was running perfectly.So after 5-6 mounths the error appeared again.I get this error,when i install my drivers for the graphic card it crashes all time can't boot.But if unnistall the drivers it runs but when i install the drivers and after that put he resolution or restart the pc gets the error all time can't boot.I haven't overclock my pc all fine.Specs below....

Motherboard:MSI 880gm E41
Graphic Card:MSI HD 6850 (not overclockec eaither,runs on 6-pin power connector)
PSU:Gigabyte 450w(don't know the full name)
Ram:Geill 4gb 1600mhz

Please help,does my graphic card died?what is it please give me possible solutions so i could try.I know for bios and settings so you could suggest everything that would work.Thanks.
