Is my HDD dying?

So pretty recently i will be on my PC either playing games or surfing the web and then all of a sudden my PC freezes up and then i get this message: http://imgur.com/kUFAmKC
I also replaced my GPU to a 1060 6GB and load up times for windows have been noticeably slower. (About 3 minutes to get to login in screen and an extra 4 minutes until windows is all loaded)

Is this the blue screen of death? Does this mean my HDD is dying?

I have only had it for a year it shouldn't die that fast.

CPU idle: 15.9 C
CPU Full load: 32.4 C

GPU Idle: 28.0 C
GPU Full Load: 59.0 C

HDD Idle: 31.0 C
HDD Full Load: 34.0C

Well i keep my room at 68f and i liquid cool the CPU with the Corsair H100i
