Is My SSD Dead?

This is the 3rd time that I've had to reinstall windows, this time on a hdd instead of my 480GB Crucial BX200. Basically what's been happening is that I'll install windows, install all my drivers and basic programs, install windows updates using WSUSoffline, then it will be perfectly fine for a few restarts and all of a sudden it will bluescreen on a startup after the windows logo and won't ever boot again. If I try to launch startup repair, my mouse and keyboard won't work and I can't select anything. Does anyone else have a fix for this before I RMA my ssd?

EDIT: Everything was perfectly fine until a couple days ago when one day it just blue screened when I tried booting windows

Download Crystaldiskinfo and run it to get a report of the SSD status.
