First I apologize for the story but I find going into detail with these problems can help save everyone some time. I've done every troubleshooter that I can find and all I can think in the end is I've either fried some of my USB devices, my MOBO has issues and might almost be dead or Windows 10 just will not work with me.

My Build:
Windows 10 Home 64bit
AMD FX-8350 Black Edition Vishera
ASRock 990FX Extreme4 AM3+
Radeon Sapphire R9-290
CORSAIR Enthusiast Series TX750 watt
Corsair h115i cooler
G.SKILL Sniper Series 16GB (2 x 8GB) 1866
Samsung 840 Pro Series 2.5-Inch 256 GB SSD
Creative Sound Blaster Recon3D THX Sound Card SB1350
Zowie Gear EC1-A mouse
Corsair K70 keyboard
NZXT Phantom 410 case

Ever since I installed 10 a few weeks ago I keep running into...bizarre problems. The first one was somehow my boot mode got changed back twice from AHCI to IDE mode. Happened once and I rolled back a W10 update and problem solved. After that my USB 2.0 devices no longer are being recognized and getting the same error from all of them: Windows has stopped this device because it has reported problems. (Code 43) (Device Descriptor Request Failed). I had tried them on both the I/O ports and Front Hub. Last week I plugged in a wired xbox 360 controller that used to work just fine on W7 but 10 couldn't recognize it. Then yesterday my webcam wasn't working and now none of my 3 mice (all working before this) will work either. (the webcam and mouse were plugged in the I/O 2.0 ports)

Things I've tried:

  • Updating all my drivers
    Rolling back Windows updates
    Reformatting and fresh Windows install
Turned off the USB power saver in each Root Hub
Individually uninstall/reinstall all USB and USB Controller devices/drivers
    Turned off Windows Quick Start
    Flashed the latest BIOS
Turned BIOS Legacy settings both on and off
    Windows Device Repair
    Manually Installing the Devices
    Installing Drivers from earlier OS's
Hard resets with unplugging the PSU, draining any extra power and manually reset the BIOS

I'm sure there are a few other things that I've missed as well.

However I did notice while I was in my case that I made a rookie mistake and plugged the Front USB Hub into the IEEE 1394 slot (exact same plugs just different colors and right next to each other) back when I had everything disassembled a month ago. So my sinking suspicion is anything I plugged into the Front USB Hub most likely got fried. I've since unplugged the USB part of the hub and just left the HD Audio plug in. It's just bizarre that it took so long for the problem to finally show itself if that's the case.

Here's where things get weird though. My main mouse (Zowie Gear EC1-A) will work on my roommates W7 computer but not mine nor my Vista laptop. The 360 controller won't work on anything including a xbox. My other wired mouse no longer responds at all. However, when my roommate let me try a cheap wired mouse of his on my PC, that works just fine (I/O 2.0 ports). Also this whole time my keyboard has had no issues but that is running off the USB 3.0 ports. Everything else still works just fine...

I'm at my wits end on this and feel like a new MOBO is probably the solution but before I go and drop the money on one I'd like to hear if anyone out there has any other ideas I can try.

Thank you

plugged the Front USB Hub into the IEEE 1394 slot
That is the problem, the USB2.0 devices ( maybe the USB2.0 ports too) are gone, because over current.
IEEE 1394 Max. current 1.5 A https://en.wikipedia.org/wiki/IEEE_1394
USB2.0 ---- 0.5A/ USB3.0----- 0.9A https://en.wikipedia.org/wiki/USB

So that will cause the MB USB2.0 ports or USB2.0 devices problem. So you may try use other hard drive to clean install the window again, and try some of usb devices to see what happens, if you got the same so that means the USB2.0 ports/wires have problem.

Thank you for your response,

I ran a clean reinstall 2 days ago and it was after that, my roommate had let me use one of their extra mice and that one has been working just fine on the I/O USB 2.0 ports. My next test will be to take my (personal) Logitech G500 mouse from work tomorrow and plug it into the back to see what happens.

If it works just fine then should I just move on and learn from my mistakes? Or would it be wise to get a new motherboard regardless?
I also think it's safe to assume I need to replace my front NZXT USB hub?(the one I had plugged into the IEEE 1397 slot)

Well after testing my backup mice on my PC everything appears to be back up and operational. System accepted the mice via the I/O 2.0 ports and installed the drivers with no problems. figured I will just leave the front USB Hub unplugged but continue to use it's HD audio for my speakers. Now I just have to replace all the peripherals I burnt out trying on the front hub and things should go back to normal.... Hopefully I won't see anymore issues regarding this. If so then I guess I'll know to replace the MOBO. Time will tell.

Thanks for the help Cin19
