Graphics Card Help on 7 y/o hardware

Having some Graphics card crashing/freezing problems and looking for suggestions on a replacement. The card is only about 3 y/o and has worked great until here recently like it was holding out for me cause it knew i was going to get some Christmas cash... but i digress...
my budge is around 200$ but it's not a hard cap.

For ~200, the best card available is the RX 480 4gb, if you are on neweggs newsletter, they have a few on sale for under that. Below that is the 470 and below that the 1050ti.

Depending on what CPU you have, you should be good up to a 1070. With the setup in my sig, I have been able to run everything so far on High+ settings. I run Forza Horizon 3 on all high settings and Fallout 4 on high (@1080p).

From Nvidia you're looking at either a 1050ti or a 1060. 1060 obviously better. AMD, you're looking at an RX 480. What CPU and speed? Obviously games that are CPU-demanding will bottleneck a bit more. 4ghz+ of the first-gen nehalem/westmere i7 still does well-enough.
 
Considering the age of your system vs the age of your 660, have you considered it might be something else, like your power supply? Anyways, you could move up to an RX 470 or GTX 1060 3gb. Either of those would be a step up for you. I'm assuming your CPU would be able to hold its own.
