Failing Hard Drive?

In most games I play, I have been getting high fps, but my game will constantly stutter and sometimes freeze for 1-2 seconds. These games include GTA V, ARK, and Far Cry: Primal. I tried troubleshooting this problem, and when I looked in the task manager, my disk was always at 100% usage. I have tried disabling Super Fetch and Windows Search, and I even did a repair install of windows, but my hard drive is still always at 100% usage. Could this be a sign of a bad/failing hard drive? I have also done a full scan of all of the sectors and nothing showed up as wrong.

My Specs:
CPU: i5-6600k
GPU: GTX 950
HDD: Toshiba DT01ACA100
8GB RAM
PSU: EVGA 80+ 500W

What is your OS?

Can you post a screenshot of the SMART report? Test it with some of these tools: http://www.tomshardware.co.uk/forum/282651-32-best-diagnostic-testing-utility
If there's something wrong with the drive, just back up the data.

If the drive is in a healthy state, then most probably, the high disk usage is caused by OS services. Depending on the OS version, you will need to some troubleshooting, as there are multiple causes and different fixes respectively.

It would really not hurt to attach the drive to another SATA port with different SATA and power cables.

Hmm, I've been thinking about that, but I should have mentioned that my hard drive is at 100% usage at 5mb/s, sometimes at 0.1 mb/s! That isn't normal, right? My HDD is a 7200rpm hard drive.
