PC keeps freezing

I have a not so old rig with a new gtx 960, amd fx6300, and 8gb of ram. I have been experiencing some issues while playing games or having several tabs open on google chrome. I have been running several programs to monitor my cpu, gpu, disk 1 and 2, and memory. I noticed that when my pc freezes, my Disk 1 ( I have chrome and windows on it) is at 100% active time. I think the problem can be related to the programs or my ssd. But I also tested my gpu to see if it is running normally. I also reinstalled all the new drivers for my card. I don't know if these results should shock me but when I ran unigine heaven benchmark, my 2560x1440 benchmarks were better than my 1920x1080. My pc keeps freezing and I really need some answers.

Then you may want to check for viruses and get the drive makers tools to check if the drive is going bad. Other possible cause is data corruption which you may want to do a clean reinstall of windows.
Check your memory usage. My guess is your went over 8GB and your drive 1 has the page file. IE your using your drive 1 for virutal RAM and it cant keep up. I suggest A 16GB RAM upgrade if you have 2 free DIMM slots. IF not and your system has 2 DIMM slots with a 32GB max then get 32. I upgraded to 24GB's last year and it made my system run like a champ again.
