Possibly an issue with the USB connection. Hard to say, but here's some things to try (not necessarily this order):
1. Use the controller on a different PC (may need to add drivers depending on Windows version and whether it's wireless or not).
- and try a DIFFERENT CONTROLLER for your PC
2. Run Memtest86 for a full pass www.memtest86.com
*recommend DEFAULT CPU (no overclock) and XMP set in BIOS (or comparable for AMD) until issue is resolved don't OC the CPU.
3. Run CPU diagnostic. For Intel:
https://downloadcenter.intel.com/download/19792/Intel-Processor-Diagnostic-Tool (must reboot if you have overclocked, but if you did I'd put back to default).
4. Try different USB ports. If using the back, just switch to the front if connected as it should not be electrically connected.
5. Use only one monitor
6. Try Steam and a non-Steam game if possible. (if no non-Steam game then install KODI and use the controller to navigate that)

Please also give:
- motherboard
- CPU
- Windows version (W10 Home 64-bit?)
- wired or wireless? (if wireless is it the original or is it a knock-off USB adapter?)
