My PC isn't loading the graphics correctly, fuzzy textures

My PC has only recently had this problem. I custom built it, 16GB RAM, R9 390, Intel i5 4690k. It will launch the game sort of slower than usual with stuttering. The recommended settings stay the same (for GTA V as an example, though the same can be said for numerous other titles) and upon playing the game, the FPS is high. The graphics however suffer. The ground looks flat, graphics look fuzzy, items take a long time to load in, atmosphere looks foggy, etc. I don't understand it. The settings are maxed out. I even turned down the settings and the same can be said. I tampered with them with no luck. The same can be said with Fallout 4. And with CSGO, it will stutter whenever i pause it and stutter every few minutes. And there is a lot of screen tearing. I absolutely refuse to use VSYNC as i am a competitive player and require the extra split seconds for reaction time. Anyways, any suggestions?

List your full PC specs. Make sure all of your drivers are up-to-date. Update your BIOS. Download MSI afterburner and check the GPU/CPU usage and temps. Download a tool from your HDD/SSD manufacturers' website and check if your HDD/SSD are healthy. While gaming, check the usage of your HDD/SSD.
