Which of these most likely causes freezes?

So, I built new PC, and am getting midgaming freezes/lock ups when gaming.
I changes PSU, and MOBO, Freezes still occur. So only CPU, GPU, and RAM left.

No overheating issue, also I ran RAM check with win7 built in diagnostic program, no errors.
I stressed both CPU and GPU, all fine. Lockups only occuring when gaming, sometimes few minutes on, sometimes half an hour after. Tested games: Subnautica, 7 days to die, Darksiders 2, The long Dark. Checked my HDD, no errors.

System:

win7 pro
GPU HD 6750
MedionPC MS 7646 v 1.0
AMD II x4 B97
Corsair cmpsu 400 cx
6GB of random sticks DDR3

Well, anyway guys. I found out that its my GPU that was causing this, as I tried it on another PC and it was giving freezes/black screens there aswell, on same games. Not sure what is wrong with it though, since in windows,applications, youtube videos etc. is working normal.
If you got any suggestion what I could try that would fix it, I am glad to try, since its pretty much useless for games now. :)

Yes, This happened before on old rig, where I had two PSU's powering system, second one was only providing power for 6 pin connector on GPU, so I got this one, and its still happening. It is 400W with 30amps on 12v rail, so power should be sufficient for my system. Idk about its integrity though.
 
1. Tested the CPU and PSU with what ? Using what to monitor ?

2. First step should be to look in Event Viewer and see what caused the freeze.

3. Not found Windows based RAM check to be reliable; if want to be sure, use Mtest86+ from memtest.org

4. Run Furmark and monitor temps AND voltages with HWiNFO. In addition to Vcore and DRAM volatges, check voltages (min and max) on the 3.3, 5,0 and 12.0 volt rails. Have seen several posts here where either min or mac was outside 11.4 - 12.6 range. Per ATX standards +/- 5% is the max. Anything worth having will deliver < 2%.

5. Corsair CX series is Corsair's "Builder" line (read low budget)

1.)I stressed GPU with Furmark it remained stable at not going over 66C, I used HW info to monitor temp , GPU voltage is 0.95-1.1V
I used CPU-z built in option for CPU stres, also stable, temps not going over 50C,voltage 0.95-1.4 depending on load.

2.) no error in event viewer to be found.Although, before fresh OS reinstall some games were freezing my PC, but some would just get "Display driver has crashed and recovered succesfully" message leaving game not responding, and in one of them I found crash report, saying that atidxx64.dll caused acces violation. And this would happen after few seconds when playing Subnautica.

3.) I will try memtest tonight, will let you know if that helps.

4.) About voltage readings from 3.3V 5V and 12V rails, Can you please advise me which software to use to monitor that, cus HW info, HW monitor, speed fan, dont show any of those for me.

5.)trying new psu is out of option for now :/

last 3 on screen ... run Furmark / RoG Bench for about 20 minutes and see range from min to max.

+3.3v

+12v

The 'recovered successfully doesn't always happen.... this is a Windows Timeout error issue and you can edit the registry to increase the time before it triggers the error message.

I already tried edditing windows registry, setting it up for longer time, but drivers kept crashing after second or two, after freeze. I reinstalled OS and now it just freezes though.

And like I said, I already checked this, since I use this program for a while, it just doesnt show voltages like that.
