2 intake at front, 1 exhaust at rear, 1 exhaust at top. don't overdo it on the top fan. you want the air to have a straight path in and out of your case, otherwise you get a vortex of hot air spinning in your case

There are actually 3 ways of doing this :
Negative pressure : all fans blows the hot air out
Positive pressure : all fans blows the cool air in
Hybrid pressure : front fans sucks cool air and rear fans blows the hot air out (classic way to cool a computer)

I would go with a hybrid pressure setup.

Yep. And if you have a vertical CPU cooler, make it blow towards the bottom of the case so the hot air gets immediately sucked out.
 
Positive pressure will keep your case cleaner.
The key is that all intake air comes from one source and is filtered.

Put two intakes in front with a filter.

All that comes in the front will exit somewhere.

Add a single fan in the rear, mainly to direct the airflow.

If you have too much exhaust capability, it will draw in unfiltered air from adjacent openings.

Your case is designed to have a specific airflow and noise level, keeping the stock fan configuration of your case is the best idea.
If your fans makes too much noise, just replace them by Noctua equivalents.

Put 2 on the top, 1 on the rear and 2 on the front (assuming it's a totally basic ATX case).
EDIT : the rear and top ones must blow the air out and the front ones must suck it inside the case.
