HDD Usage at 100%

Hi techies, I have a build that I finished about four years ago, and I believe it is dying. I have a refurbished 750GB, 7200 RPM HDD that seems to be failing. I recently booted up this computer to find that the drive was not even read by the BIOS/motherboard, so I opened the case, unplugged the drive and plugged it back in. Both SATA cable and power cable while the computer was on. Then I restarted it and it worked fine, with the exception of 100% disk usage. I have been having the disk issue for the last two months, but it didn't really seem like an issue at first, but I know now that 100% disk usage should almost never happen. I also have issues running 7 Days to Die, but I'm not completely sure if that is the drive's issue. I meet all of the system requirements for the game, but it still runs poorly. I think this is due to the fact that my drive cannot access the textures fast enough.

CPU: FX-4300
GPU: GTX 960
HDD: Seagate 750GB, 7200 RPM
RAM: 8GB G.Skill @ 1600MHz
Case: Xigmatek Assassin II
PSU: EVGA 430W Non-modular
Motherboard: MSI 970A-G46

"System" is taking up all the resources, but it says it is only writing 2 MB/s... System is Windows, but I never had any issues with that in the past. I'm 100% sure it's not SuperFetch either. I'm also on Windows 10. When Steam is installing a game, the drive can go up to 30 MB/s and still no performance loss.
