Is my hard drive failing?

So my pc has been running very slow recently and i honestly think that it is my hard drive since it is a toshiba hard drive and I've had it for around 4 years. So far I am planning to buy an ssd and a new hard drive which is western digital blue.
Specs:
AMD-FX4300
PNY GTX 960 4GB VRAM
8GB RAM

Probably 4 years is about time when things start to go wrong.
First sign is slow disk access.

Do a backup. Running slow is kind of a broad generic term. No doubt upgrading to a SSD will speed some things up.

I meant do a backup to make sure you don't lose anything when the drive fails or when you swap it out, quite a few folks won't bother.

Have you run diagnostics on it? Do a backup first
