Lines all over the screen on desktop PC monitor possible GPU failure?

Hi i have been having problems with my PC lately when i start it up and log in i get lines all over my monitor. i diagnosed it a few weeks back when it happened by switching ends of the HDMI cable going from the GPU to the monitor and unplugged and plugged back in the GPU into its pci express slot and it worked fine. Now here i am this week and the same thing has happened but.....this time i diagnosed it similarly to the last time and it has not fixed the problem it is looking like a GPU problem to me personally as i tried running GTA 5 with the lines all over the screen and i was getting literally 1 fps an i closed the game down i also tried running furmark and it wouldnt run at all my motherboard and operating system and nvidia control centre still detect the card despite these problems. Any help is much appreciated thanks.Below are the specs of the PC:
Intel I5-6400
16gb ddr4 RAM
msi h110-m proD motherboard
Gainward Nvidia GTX770 graphics card
bitfenix neo case
750w 80plus bronze PSU Aerocool xpredator

The causes a Gpu chip that is running far to hot even at idel.

Or graphics card memory on the graphics card begging to fail.
That may be due to dry or bad solder joints on the under side of the memory of the card where it solders to the circuit board of the card.

If your getting a low frame rate in games, and the GTX 770 card is of the reference cooler type with the blower type fan configuration, you need to remove the cards top side cover where between the blower type fan and the front edge of the cooling solution fitted to the card you will find a thick matte of dust blocking air flow through and along the aluminium cooler fins of the cooling solution on the card and exiting out the back of the card PCV3TERAN.

Take the top cover...

Take the top cover off the card and check, it`s normally the case that it`s blocked by a thick matte of dust buildup !
