GTA V unable to initialize DirectX 9

Solution
It`s more than likely that if you have something like a Intel board with on board graphics output of the motherboard that GTA 5 may be detecting that graphics solution.

Go into control panel of windows if the case.
And then click on system and security.

Then click on device manager.

Left double click on display adapters in the hardware list.
You should see the GTX 960 4Gb card listed.
If you see the Intel integrated graphics solution also listed as running or working.

Right click on it and disable the hardware.

Then try to run Gta 5 again Xephobia.

You other option is to go into the bios settings of the motherboard.
And ensuring that the Pci-e interface for the GTX 960 card is the only one enabled and selected. Interface mode =...

Some motherboards allow you to run both the Pci-e based graphics card along side the integrated Intel graphics solution of the motherboard for multi monitor support So you should disable that option in the bios if it exists along with Igpu memory

Render standby mode.
And Igpu multi monitor support.

Could you please list the brand of motherboard you are using and it`s model number then i can check for you if its the case of the Pci-e card and the Intel graphics solution of the motherboard running at the same time xephobia.

On a 64 bit system, it will be here:
HKEY_LOCAL_MACHINE\SOFTWARE\Wow6432Node\Microsoft\Direct3D\Drivers

But first try Sean's suggestion. It may be that your on board video is being picked up first

If that doesn't work, go to start, run " DXDIAG "
That will bring up direct-x diagnostics. Check what video card it is using under "display 1"

If your on board card is shown there, but you are definitely using the add-on card, disable the on-board:
