Micro SD won't work

My 8GB micro SD card won't work properly on my android. I created four folders on it, but when I look in the phone, there are just two of them. Then I plug phone in computer and there also are just two folders. I tried to add photos and music to those folders but it also won't show up. What's the problem?

Flash memory, be it an SD card or a fancy new SSD, only has a limited number of writes before the memory cells wear out. Perhaps yours is old enough that it has reached that point and needs replacing.
