Black screen with max GPU fan speed

While playing a game about 20 minutes ago my Pc black screened and the fan speed maxed out.
This is the 3rd time this has happened in the last 2 months, but it worries me so i want to see if there is any fixes. I have already tried a few solutions I have seen on the topic since it happened last, but as it just happened again, none seemed to have worked.

For more info, when this happens, all my temps are in the normal range, and it has happened before when just web browsing, so it is not always during intensive activities. My guess would be a psu problem, but I am not an expert and I would like to check before making assumptions.
My pc is not overclocked, and I do have one of the 1080FTWs that had the heating problems, but the thermal pads are on and correctly installed.

Edit 2/25: While playing ffxiv, this has happened 7 times today, more than the combined total of all times in the last month. I tested the game in both dx9 and dx11, and both have crashed. In the Reliability Monitor, all the crashes had the exact same error:

since it work back after sleep i would try to switch power cable from psu from cable 1 to cable 2 also your monitor is able to handle the scren resolution you ask if you could try another monitor for test to see if it not him that cut the gpu power ,if all those thing does not work you will have to contact evga for a rma .

Is the problem with the psu something fixable, or should it be replaced? If I should replace it, what would be the optimal choice to buy?

(note: It crashed again in an intensive part in a game, so afterwards I tweaked some things to lower power consumption, then tried the game again and had no problems. As I can't tell what is wrong with the psu based on readings, is it just going out?)

I ran the program with the log function while a game was on the Title Screen for about 20 minutes.
Right after I logged into the game (Final Fantasy XIV) my pc immediately crashed with the loud GPU fan sound and black screen. I had the program recording a log, and I don't know how to open or view the info on it, but here is the .csv text file to it: http://www.mediafire.com/file/aykfx2d9a6uynzi/pclog2.CSV

Other notes: I have the game running in dx11, and have mostly only had issues while playing this. These issues only started happening after I upgraded my monitor from a 1080p to my current one listed in my original post.
I have gsync enabled, and nvidia settings were set to this at the very last time it crashed: http://i.imgur.com/WHdOMB2.png The only difference is I had Vsync set to "fast" this time, as I have been changing and lowering settings every time it crashes to try and find anything that might fix it.

I was running the game in dx9 when it crashed, and had no overlays or other programs running.
I can't scan the actual memory dump file without buying the pro version, which I won't be able to do until I get paid next, if I need to. I could also upload the entire memory dump file, but the file is huge. (900mg)

Edit: I would also like to point out that the pc doesn't blue screen, It just goes black with fan speed maxed, ect until I hold down the power button to turn it off.

Edit 3: I ran Driver Verifier and restarted and was presented with the "DRIVER_VERIFIER_IOMANAGER_VIOLATION (ScpVBus.sys)" BSOD. I have noticed that the game that I play when I crash uses a ps4 controller which I had previously always had plugged into the back of my pc, but I had very recently moved it to a port on the top; could there be a problem with the controller or port on my case?

[strike]What should I do with it, delete the driver for the ps4 remote? If I do, will it automatically reconnect the next time i connect the controller, or would I have to find and download something for it to work again somehow?[/strike]
Edit: Changed usb ports, uninstalled the driver and re-installed by plugging in controller. PC crashed again via testing game, however I checked Whocrashed, and there was no record of it in the crashdump file whatsoever; the last crash is the one from yesterday.

I do have one of the "overheating" 1080s, but I have installed all the thermal pads provided, so idk how overheating could be an issue. Should I ship my card back and seek a replacement, or is it possible it is another issue or a driver problem somehow? I can run hwinfo64 again and try to make it crash again, if that would help narrow things down. The last time it crashed was within 45 seconds of booting up the game btw. It seems to crash far more often in high-population areas with a lot of things rendering on screen.

Evga released thermal pads to apparently help with the overheating issue, along with a bios update which I have already applied. The pads I have put on are shown here: https://www.youtube.com/watch?v=8DQYP0SN_Kk

edit: I also contacted a computer store in town, but after letting them know that it happened more when i upgraded my monitor, their answer was to downgrade monitors back to the 1080p one I have; which isn't exactly the most helpful solution.

This time I let the pc run for about a minute after the display crashed. The pc stayed on with the fan speed maxed, so I decided to try and press the "sleep" button on my keyboard, which worked and put the pc to sleep. when I woke it back up, the monitor stayed off with no signal. I tried changing the connection from display port to hdmi to get it to connect, but the display was totally dead, even though the fan was back to normal after waking it from sleep. I had to power the pc down and back on, and it bootad normally with the display coming back on.

I can tell mine are definitely my GPU fans, and yeah the screen goes black also and the only way to fix it is to hold the power button until it hard turns off, which hurts every time lol

Googled the issue and reached this topic. Own a Asus rog strix OC 1080TI and I'm experiencing the same issue. Will try a few of the things mentioned in the post myself. I recently upgraded from a 2600K to a Ryzen 5 3600. The wraith stealth cooler is absolute rubbish. But yeah I think this issue is related to something about the graphics card. Not sure what yet. As the card doesn't even get that hot when it does this. Could be vrm's.
Oh and this problem occured on my 2600K rig as well. but lately is is happening far too frequently. As in happened twice in a row last night. Guess I need to improve my case thermals maybe...
Own a Fractal design R5, with two 140mm intake fans in front and one 140mm fan in the back.
Maybe not enough exhaust?
