100% Disk Usage at all the time

Hey guys. I've been facing 100% disk usage problem for quite some time. Recently it has been unbearable. My PC slows down and when I checked task manager I found disk usage was 100%. This happens every time I


    ■ Start my PC
    ■ Play a video
    ■ Open my browser
    ■ Try to multitask (Play a video and browse)
    ■ Download with IDM (When it's rebuilding)
    ■ Copy-Paste something


My desktop configuration is as follows

  • ■ Intel core2duo 2.93ghz processor
    ■ Intel DG41RQ motherboard
    ■ ATI radeon 5450 graphics
    ■ 4gb DDR2 ram
    ■ Samsung 500gb HDD

I've tried disabling all the unnecessary services. But none seems to work

I'm using windows 8 but this problem also persists in windows 7 and windows 8.1. There are also totally random blue-screen errors.

Please help me out. TIA

run windows disk defragmenter. hit start and R at the same time, the type 'dfrgui' (without quotes) then hit enter. one by one select a disk, and click 'defragment'. this may take a while. i would do this once a month to keep things speedy.

I also do this regularly with IOBIT smart defrag or Default windows tool but no change.
