What do I need to upgrade in my PC in order to play games better?

My components:

CPU - Intel Core I5 6400
Motherboard - Gigabyte H110M-S2H (Socket 1151) Micro ATX
RAM - 1x 8GB DDR4 2133MHz
GPU - Nvidia GeForce GTX 750 Ti
Hard Drive - Toshiba 1TB SATA 6GB/s 7200 RPM
PSU - Really Cheap 750W (Will be buying a more reliable 750W soon)
No SSD

Now I'm not sure why but some games like H1Z1 and Battlefield 1 I can't run too the standards i want, also sometimes my PC seems to go under a lot of stress and freezes, when i check task manager the CPU peaks to 100% but then goes down to 40%. The same happens with the Hard-drive sometimes, it goes to 100% then goes down again and the PC runs fine after that.

Thank you in advance for any replies that can help me fix the problem above or help with some tips on what to upgrade next.

If your budget permits: GTX 1070 + 16GB (2x8GB) RAM + SSD

Sidenote: If you are going to upgrade your PSU (as you mentioned in your post), I'd just get a good-quality 500W-550W PSU. No need to get a 750W as that is absolute overkill. Get a Seasonic G-550 or the more affordable Seasonic M12II-520 or S12II-520. That would be MORE than enough for the upgraded GPU.

Thank you for the graphics card and memory upgrade tips guys, will look into the 1060/1070 series and getting increased RAM. Also will look into getting a 550W PSU didn't know i could run the PC with that sort of PSU. I have 1 more question which is what sort of size for an SSD? Would 120GB do if i have Windows 10 and want to put a couple of games on it for increased performance?

I agree with the 1060 6gb and an extra stick of 8gb ram, but I would also recommend an SSD to run windows. you'll be surprised how much an SSD will speed up your PC, you wont regret it. it doesnt have to be extremely expensive either, a samsung 850EVO 128gb or 250gb. good luck!
 
For SSD, I would get Samsung 850 EVO 250GB.

Depending on how large your game files would be, number of games you wish to install (and how you organize other files in the drive), I find the 120GB as too tight. I am using a 120GB in one of my PCs now, and with the OS (Win 8.1), several programs (MS Office, PS, SU, CAD, etc.), and utilities, I find that I can only put 3 to 4 AAA games in that SSD (with at least 15% remaining GB not used). The rest are in my 1TB HDD.

Also, as far as Samsung 850 EVOs are concerned, best price/performance is the 250GB compared to the 120GB.

Add: If your budget permits for a Samsung 850 EVO 500GB (or even 1TB) - that would be the best option.
