I've had this card for 2 years and June of last year, I sent the card in for rma because it stopped outputting video and was no longer detected by the motherboard. Also the fans seemed to be spinning at max rpm whenever it was plugged in. It came back fixed but now it's doing the same thing. Both of the times this problem occurred I had been afk so I can't say what events happened before failure other than the pc was idle. I still have 3 months on the warranty so I'm waiting on approval to send it in. If anyone has any clue as to what could be happening or even a solution that doesn't require me to rma, I would like to know.

System spec are:
i5-4670K
MSI Z97 PC Mate LGA 1150
Kingston HyperX Blu 12gb (4gb + 8gb)
Thermaltake TR2 TR-600 600W ATX12V

It sounds like it would just be a bad GPU. I have the same card as you and have never experienced this before. You could try reinstalling the driver to see if that would help or try putting it in a different slot, but I think it will just be a bad GPU.
