Seagate Hard Drive Max Usage on Boot

My disk will spike to 100% usage when it should be idle. It does this at boot up and can stay like that for hours sometimes jumping from 25 - 100 back and forth. Here is a snip of all my disk specs:

To avoid redundancy here is what I have done so far

- made sure my disk driver was up to date
- ran an error diagnostic check on my drive - no results
- all of my SMART info checks out ok
- my disk boots properly in safe mode with networking, but I am not sure how to monitor the disk usage in safe mode, as the task manager does not show disk usage in safe mode
- tried a different SATA port, which gave me "disk needs to be replaced, press any key when replaced" error or something to that effect, when I pressed "any key" it started just fine, but still at 100% usage on boot.
- it is not hot or loud, no cables are loose
- disabled superfetch and wsearch
- changed my power options to performance mode

I am at the point where I will buy a new harddrive, possibly a solid state if this doesn't stop as it is really the only thing slowing down an otherwise fast system. This is what I may be forced to do. Any tips would be helpful

I finally go a chance to do so. The HDD is dying. Nothing I have tried has worked, even a reformat. However, I went out and bought a Samsung 850 Evo SS. Buying an SS drive is the most well spent money I have ever put towards a system. WOW.
