GPU failing while gaming, trying to diagnose culprit

I use a mid-range gaming PC which has held up well in the past but has recently started to crash while playing even non-demanding games like Skyrim, which crashed it within one minute of the game starting. I've seen two types of crashes, which include:

  • ■ The screen turns off or goes black and the PC doesn't respond to input, forcing me to reboot.
    ■ The screen turns off for a moment, then turns back on and the game resumes.
Strangely, performance before the crash is exceptional with a steady frame rate. I know that this isn't a heat issue, as my case is very well ventilated and the card rarely exceeds 90C. I changed the thermal paste not too long ago as well. Just recently, I did a full OS reinstall and installed all important Windows updates along with all the important hardware drivers. The crashes still persist. I've considered that this is the fault of my GPU or PSU but I need help finding out what component is behind this before I buy a replacement.

Sorry about that, my PSU is a Corsair TX750w. I just played one of the few games my PC can handle (Stalker CoP) with ultra settings and enhanced full dynamic lighting + max AA. I got 60+ FPS and my CPU temps maxed at around 74C and my GPU maxed at 68. Hope this helps.
