GTX 1060 Fans turning all the way on and screen going black

Whenever I try to start a game my GPU fans go to 100% and my monitor turns completely black. Once this occurs I have to forcefully shut off my computer. I have only tried reseating my GPU and that did not work.
 
Solution
My questions to help figure this out is: Are the drivers up to date? Can you even reach task manager when the screen goes black? Does the integrated graphics work when the screen is black?

My drivers are up to date and I cannot reach task manager while the screen is black. Also, integrated graphics do not work when my screen is black.

GPU temps before starting a game are around 42 degrees Celsius to 50 degrees Celsius. Nothing in my computer has been overclocked.

Hi, same here brand new 1060 from zotac and if you game for 45 min or somthing, all fans in the whole maschine turn to 100%
and both monitors i have, turn black (no video connection).
Ive tryd strestesting the gpu and the cpu, everything works fine and runs for over 45 min.
I also tryed to disconnect one monitor and it woked fine so far. Will let u know if that changes.

System:
CPU: i7-6700K
GPU: gtx 1060 amp! Zotac
MB: ASRock z170 Gaming k4/d3

I had some issues with gpu's just like you had. First thing you need to do after installing your gpu is to reinstall your system. Let's be ohnest that even ddu program doesn't helps as much as installing new windows. It needs to update all drivers. Start from reinstalling windows. If you did it and it doesn't works , plug one monitor via DVI-D connector and check the performance. Than check just one monitor with HDMI. If everything works , than your card have problems with managing two monitors (this is unusual but it happens) , than RMA your card if this is the case.

My father has the same issue on the exact same card. This has been identified as a Hardware failure of the GTX 1060 AMP edition. speak with ZOTAC and get it replaced under there 2 year warranty, but before you do that, speak with the retailer first to see if they will replace it.
