Fallout 4 GPU usage goes down over time, causes low fps

Hey guys, this problem has me completely stumped and I've tried a myriad of solutions to no avail.

The problem: When I first start up Fallout 4 (modded to some extent, but nothing extreme) the game runs perfectly smooth, usually capped at 60 frames as I have tried limiting it and using Vsync to resolve this issue. However, after a while it seems like my GPU usage goes down significantly. From about 50% right after loading a save to about 15-20% in the end, causing me to have a very low fps of about 20 and even lower in the settlements that usually require a lot of performance.

Workaround: I've so far worked around this problem by simply restarting the game and loading the exit-save and the game runs smoothly again for a while. Now, it seems to me that if I enter a major settlement that is really resource-heavy the GPU usage goes down and after leaving this area and returning to any other location will still yield a low framerate. Though this might just be my confirmation bias working against me.

"Solutions": I've downloaded a few mods that promise to help with performance issues like ShadowBoost for example, that simply reduces the draw distance of shadows as the framerate goes down. Texture mods for trees and rocks that are, according to the author, more performance-friendly than the original ones. I tried changing a bunch of settings that people say might improve performance in the .ini configuration files. Nothing has worked though.

I should also mention that I have changed the default FOV to 90 instead of the default 70. Turned of godrays and set shadow quality to high rather than ultra. Lowered the draw distance of actors and objects as well as their distance quality. Tried adaptive V-sync, no V-sync and normal V-sync in nVidias controlpanel as well as the "Prefer maximum performance". Tried running in borderless windowed mode and also in fullscreen.

Now the question is, has anyone ever encountered something similar and do you know how to fix it? It seems to me like there's definitely enough "power" in my PC to run this game no problem, but the game isn't using these resources.

i7 6850K 3.6GHz
Zotac GTX 1080 amp extreme
Game installed on an SSD drive
64 GB RAM
Windows 10 Home

Quick edit: I can play other games perfectly fine, Battlefield 1 runs on the highest settings without any problems.
 
Solution
Check nvidia settings for the game, and select preformance, also check windows power settings and leave it on max performance or similar. Hopefully the issue is there.

I wish it was. Unfortunately I've already tried the "prefer maximum performance" in the nVidia settings. And I've made my own setting in windows, though I will try switching to high performance instead.

Edit: After some time now I tried the high performance mode instead of my personal mode which I am pretty sure was already set to high performance. I also apparently had a DVI cable connected to only my monitor (I use DP) and had left it connected, so I removed that cable even though that shouldn't cause any problems.

The most significant thing I did however was to first shut down the Shadowplay "replay" function and then shut down GeForce Experience entirely and so far I've been in game for over 2 hours without noticing any signs of lag. Not even in my modded Sanctuary settlement where I have modded the size limit so that I could add much more items to build a huge mansion/warehouse and city wall with the Sim Settlements mod adding loads of small structures as well. So far everything seems to be running smoothly!

Because I can't be entirely sure what actually helped, I will mark the previous comment as the solution.
