Can I Run Gta 5 on High Settings?

At 768p i'd use most the latter settings still but consider turning up some other details to High such as Ocean quality and reflection quality. Other than that you could try turning up AA to MSAA 2x (Sharper) and AO to HBAO (More realistic and accurate lighting) and that should give you good performance with nice graphics quality. Just experiment for yourself once you have tried the above settings to see if you can handle any more higher details.

You should be able to sustain a combination of settings on Medium/High. Just try avoid any form of Advanced graphics settings and keep AA + Filtering to lower (such as FXAA for Anti Aliasing and AO on SSAO at most). As for the settings, i'd keep most on Medium with maybe High Textures + Terrain but leave everything else medium to minimise FPS drops. Hope this helps :)

- My old i5-2500 and Asus GTX 760 OC can run the game at this combination with only a few higher tweaks at stable FPS in 1080p.

16GB RAM does help with a handful of games these days but a stronger GPU would be a better investment at this point. If you can aim for something like a RX 570 that would be a good upgrade, capable of most games in 1080p 60 FPS on High and if you have more an RX 580/GTX 1060 6GB can handle most at Ultra settings.
