This is a new card. The default clock speed is 1342 mhz and the default memory is 2000, but when it was set to default every game crashes in intesnive scenes, my monitor dissconects from the GPU and I have to restart my computer. I had to underclock it to 1150 and 1800 for it to work.

I5 4690K
12 GB DDR3 1600 mhz
Win 10 64 PRO
Eco silent 750W Psu

Solution
That might probably point towards a PSU issue.

"The screen goes black and monitor somehow gets disconnected from graphics card" — Your GPU crashes and is unable to give out display signal, probably due to inadequate power supply.

Try reducing the power limit of the card in steps of 5% without underclocking, till you get no crashes. Also monitor the GPU core and memory clocks during the stable run. I'd suggest MSI Afterburner for this purpose

If it doesn't work, try the usual troubleshooting steps like installing latest drivers, try the graphic card in a PC with good PSU if possible, etc.

1| You forgot to include your chassis, motherboard, the ram and it's specs and the monitor+resolution as well as the make and model of your PSU. I've found the Eco silent being made by Silentmaxx which isn't a reliable brand to invest in. I could be wrong though.

2| If the system shuts off when you're undergoing intensive scenes then it's highly possible that the PSU is giving up it's duties.

System does not shut off complety, the screen goes black and somehow gets dissconected from the graphics card and i can either shut it down on the power supply or restart it after a few minutes and then its back to normal.

I have just tried it by limiting the power by just -5 and the default clock speed works. My only question is can this hurt the graphics card in the long? Thanks in advance.

Since you are able to get stable performance at lowered power consumption, (underclocking or lowering the power limit) PSU must be the culprit.

So try to get a good quality PSU. Seasonic makes good ones. Even a 600W high quality PSU will suffice.
