Should I upgrade my radeon 7950 to a rx 580 nitro

That is still a pretty great card, a real gem by AMD. I would hold out a bit and see what vega brings. yes a 580 is a nice step up, but for the price I am unsure if it would be worth it. (although you could probably still sell that card for something decent.)

Because the games I play like fallout 4 seem to be running a little slow
the fps I have been getting around 20 fps on high custom settings

Likely running out of VRAM. a shame, because the core on that card is still capable enough do better. 20 FPS is no fun, so then I guess you are after a new card. in that case then a 580 would make sense.
