Not sure if its my graphics card or something else?

Looks likely to be a graphics card software or hardware issue.
If you get this glitch in only this particular game and other games or 3D applications run fine, then likely the driver. If all games are showing glitches then there may be some hardware problems with your graphics card .

What GPU are you using and what are the game you are having these problems in?
Also which games are running fine?
Once I know this, I will get a better picture and would try my best to help.

GPU: Radeon (TM) RX 480 Graphics
Games that I have problems on are Squad and Star Citizen.
Games that run fine are the whole Total War series Space Engineers, and Arma 3

See if some games are running fine then this may well be the case of GPU drivers and the game settings.
Try disabling Anti aliasing and Shadows and see if the problem persists.

Also, try getting the latest drivers for RX 480 from the link below:
http://support.amd.com/en-us/download

Completely uninstall the existing drivers and do a fresh install of the 16.7.1 drivers. Check if the problems are gone.

If not, RMA (return to your vendor for replacement) your card ASAP, if it is still under warranty.
