How can I force my SD card to format

Hi guys

I recently ran in to a problem with my SD card. It is a Lexar 64gb 300x 45mb/s card that came with an adaptor. I have had it a little over a year, which means the warranty has also. Just finished.

Anyway the problem is I cant format the drive and I cant delete anything off it. It started one day when I went to switch my camera on and it said that it didn't pick anything up. I can copy stuff to it and delete things off it,but if I disconnect the sad card and put it back into my computer then shows all the old stuff that was on it and doesn't show the latest changes that I just made.The main problem is I cant get the images of sd card. Basically I want to format the card and start fresh but nothing I have tried works

I have tried the following steps to get it to work:
Simply clicking format on multiple devices
Put it in another adapter to try and format it
I tried to format it in an android phone
I tried to connect it via the phone and use it as a mass storage
I have tried different laptops and desktops
Chkdsk in the command prompt
Disk/device manager ( creating a new partition doesn't work either)
Clean disk in the command prompt
It have tried the REGEDIT solution where you force it to write and read
I tried the official SDHC formatting software as well as other formatting software
I tried to format it on my canon 760D
I contacted lexar and they could not help,mainly because my warranty is over.
I tried connecting the camera with the sd card in it, to the computer and formatting it.

If anyone knows how I can force this sd card it would be greatly appreciated because I did pay quite a bit for it.
