Random Blue Screens/Restarts, Freezing, and Programs Not Being Read/Opened (Error Code 0xc000000d)

Hi everyone, I really need some help on this one. I have run plenty of tests to see what is wrong with my computer, and I just cannot find the issue. I have run MemTest86 three times, and it has only found one error on one test. The other two tests passed without an error. I have run Hot CPU Tester Pro 4 (Trial) for the full six hour test, and it turned up as everything being at 100% integrity. I ran FurMark a while back, and that turned up nothing either, so it's probably not the graphics card. My score in Fire Strike was pretty bad as well. (might just be because of my terrible system (which will be replaced this week.)) So, basically the issue is what's in the title. I just did a fresh install of Windows a week ago, and the problems went away, but of course, it's going through the same cycle of problems it did the first time. First it was the blue screens. I never thought anything of them, and I just thought it was Windows 10. Next was the issue of programs not opening, not even anything in task manager. Next were the error codes of programs not being found, but on a restart, it will read them. After the error codes started showing, and after the computer started to get VERY slow (even boot times were longer), I checked the HDD in task manager, and what do you know, it's running at 100% usage. Finally we have the freezing. I could be doing something and -- oh! All of the sudden it freezes. It used to recover, but it doesn't anymore. It just sits there frozen. When it freezes, I also notice that the HDD LED on the case is either off or solid red, indicating that is completely off or at full usage. I originally thought the issue was the HDD, so of course, I switched out the 7200RPM drive I had with an old 5400 RPM drive from a laptop that I know works, and did a fresh install of Windows. The laptop drive went right up to 100% usage, therefore confirming the hard drive is not the problem. After running Hot CPU Tester Pro 4, I though the motherboard couldn't be the problem, leaving only the PSU or GPU. The possibility that the motherboard is the issue is still in the back of my mind. I have run a 4.5GHz OC on my FX-4300 after the HDD had it's 100% usage issue, hoping I could squeeze a little more power out of the system because it was so slow. I will say that I was running that OC on stock cooling, on a very bad motherboard (970A-G46), yet the temps seemed fine in HWMonitor (45-50C idle.) After the new install of Windows, I have not OCed. I ran Prime95 for four hours as well, just for it to turn up nothing.

Mainboard: 970A-G46
HDD: Seagate Barracuda 750GB @ 7200RPM
RAM G.Skill Ripjaws 4GB x 2 (8GB) @ 1600MHz
GPU: GTX 960

onboard VRM failure on the board maybe because of a faulty choke. Check for blown capacitors or swollen and leaking ones anywhere on the board, around them should be the culprit if not the caps.

Sorry for the very late response, I just moved from Pennsylvania to Georgia. I'll check for damage to the motherboard. I wouldn't be surprised if the VRM module is the issue, as this board is known to have a terrible one.
