Can a single graphics card handle multiple monitors with different frequencies?

I have a GeForce GTX 960 video and two DELL U2412M monitors (1920 x 1200). These are 60Hz monitors. They are great monitors, but not built for gaming. They are connected via DisplayPort.

If I add a 21:9 freesync gaming monitor (to the 3rd DisplayPort output), do I need to worry about the GPU trying to refresh the screens at different rates?

Just as you can have different res, you can have different hz. The gpu knows the monitor's capabilities and sends a different signal to it. The 960 supports up to 4 monitors and has plenty of power to run multiple 4k monitors (not gaming) although that would be limited by its connections.
