something not right about my monitor or gpu... confused please help

hi, so i bought new IPS 1080p monitor ASUS VX239H and gtx 1050ti some weeks ago. But my gaming experience is not smooth when i move camera of character in the game. i play games like hitman 2016, gta v, rise of tomb raider... i face kind of unsmoothness, screen ghosting and laggy effect even with constant 55+ FPS. I dnt know if its about GPU or monitor itself. I cant game for long period because it hurts my eyes after a while.

i did a benchmark on UserBenchmark and it says that my GPU is performing below percentile. IS this a problem? BTW i bought new GPU and monitor.
please suggest me which other GPU benchmark software to use to know if my GPU is working alright?? or the problem is with the monitor i am using?? please help

Thanks for reply :) My monitor supports upto 60hz and screen refresh rate is already 60hz (maximum) in windows display adapter properties and nvidia control panel. Is there any way my monitor is physically defected and actually working below 60hz?? is there any way to find about it?

yes i am using HDMI CABLE

But something in nvidia control panel seems to bothering me, please have a look

shouldnt it be HDMI to monitor rather than HDMI To HDTV ???

no bro hdmi is input port of graphics card (like dvi, vga, display port), hdtv is output. I think Hdmi to hdtv means nvidia control panel thinks that my output source is TV rather than monitor.
