Is my SSD dead?

So my ssd been acting weird lately, I've been checking it with crystaldisk info and Sandisk's own software and it listed as Sata 3gbps sometimes while 6gbps other times(after restart). So I bought a new SSD and did a fresh install of windows there(since its faster, PCIE M.2) and the SSD still on weird switching between 3 and 6 gbps so then I thought I try doing firmware update that SSD dashboard been asking to do and that did not fix it. So then I found a setting in Intel Rapid Storage technology which is called "Link Power Management" and it's on "Enabled" by default and so I went on and disabling it. After that the SSD been on 6Gbps and I thought well that fixed it and then today SSD just disappears from windows and not detected in BIOS either, could it be dead? I tried different SATA cables and ports and its still not being detected. It is slightly a year old and still in warranty. I even try switching its place with my HDD and it still doesn't show up.

It's a 240gb SanDisk Ultra II SSD
On Windows 10 pro 64bit
Mobo: Asus Maximus VIII ranger
Speed: on 6gbps its on about 500mbps(read) while on 3gbps its around 280mbps(read) via crystal disk mark

 
Solution
Short of the power connector itself (I've seen them come loose and stop making contact on cheap PSUs), I think you've tried everything and it's safe to assume it's dead.

I tried switching it with my HDD(using the HDD sata and power that is proven working)and also didn't work. My PSU is corsair is RM750i so I guess it would have some decent connectors. Well RIP SSD then, its weird though, dying just like that out of nowhere. Before it died, the SSD Dashboard said its life is still 97%.
