GPU and case fans not spinning

For the past 2 weeks I’ve experienced my monitor blacking out while the PC ran just fine (could hear music and pause it via keyboard). Even using my TV as a dual monitor with duplicate/extended screens, both screens would still black out, and I would need to restart once or twice to get it to work. I’ve also replaced the DisplayPort cable just in case, with no such luck.

I checked my PC while it was running, and discovered that my GPU, as well as my case fans, weren’t spinning at all. My CPU heatsink runs fine, and thankfully I had the AC on so it didn’t overheat. When I power on the PC, the GPU fans will twitch but not start up, and the case fans remain still. So far I've double-checked the cables and nothing seems loose, and reseated the RAM as well.

Would this be considered a power supply or GPU issue? I’ve had to replace my PSU before due to random power outages, so I’m wondering if this would be the cause again. Here are my specs:

Case: NZXT H440
CPU: Intel Core i5 4690K
Heatsink: Cooler Master Hyper 212 EVO
GPU: EVGA GeForce GTX 960 SSC
Motherboard: Gigabyte GA-Z97X-UD3H-BK
Ram: 8 GB Kingston HyperX Fury
SSD: Samsung Evo 850 250GB
Hard drive: WD Blue 1TB 7200 rpm
PSU: Corsair CX750M

In this case, we can assume motherboard is ok. Still can't decide if fans and blackouts are two separate issues. Since fans are working, the fan hub might be a problem, or it is not receiving power from PSU? I suppose hub is powered by Molex or SATA plug connected to PSU. If yes, first step would be to use different plug/cable. If that does not help, it's either faulty hub, or maybe you connected too many power hungry fans to it? Try connecting only 3 or 4 fans to the hub and see if that changes anything.
As for blackouts, you would need to rule out bad GPU first. To do so, install it in another machine and check if it makes blackouts there too. If it works ok, then next suspect is PSU indeed.
Well, GPU issue could not cause your case fans to not work. Also, if your case fans are connected directly to motherboard headers, PSU failure could also not cause this. So either you have two separate issues or there is problem with motherboard.
My advice is to remove GPU and connect monitor to motherboard, to see if that blackout problem has anything to do with GPU.

I've unplugged the GPU and connected my monitor to the motherboard, but the case fans would still not spin. My case fans are plugged into the hub provided by the case, but when plugged directly into the motherboard, they start to work again. It's a bit difficult since I have 5 fans but only 3 available slots, but at least I don't have to worry about my build overheating as much.

I haven't experienced any black screens from the motherboard so far, either. It seems like the GPU just isn't receiving enough power, hence the blackouts-- but I'm still unsure which component is the cause.

UPDATE:
I completely forgot that the 960's fans do not spin unless they reach a certain temperature, but it still does not explain the blackout issue. I'm currently uninstalling and reinstalling its drivers to see if that helps. As for the case fans, that might just be a separate issue entirely.
