SD Card Write Protect

I have tried to format my SD Card but it keeps on showing that it is on write protect, so I tried the command prompt as well as the regedit in order to take out the write protect, but it still doesn't work. I have also checked the lock switch on the side of the SD card reader, but it's not lock. Help, I do not know what else to do. I just want to format my SD card, all of my files already has a back up. Please tell me what to do?

The card or card-reader is faulty.

Buy a new card, if that works, dump the old one.

If new card doesn't work either, buy a new card-reader.

Yes it is a Windows system and I have also done diskpart to clean the disk, but when I get to the command "clean", command prompt states that I have an I/O device error?
