Better case airflow, turn fans up if possible, clean it (probably what you should do first if you don't do so regularly).

If any of that fails, try reducing settings you are using, putting a FPS limiter on or using v-sync (to reduce the usage on the GPU), or replacing the cooler/GPU altogether.

Solution
Is your 9500GT fanless? If so, you need better case airflow.

If it has a fan, check for proper fan operation.

What is your ambient temperature?

Graphics cards do run hot, but they are built to tolerate heat. 80c. would be ok.

Try taking the case covers off and directing a house fan at the innards.
If that helps enough, look to better case cooling.

Perhaps it is time to replace your gpu.
Modern cards are stronger and more power efficient.

A card of that age may need to reapply thermal paste. Dusting the heatsink out will help as well. What are you using the card for? Trying to do anything graphics intensive on that card will stress it out to 100%. Even light gaming will overload that card.
