Keyboard powered, but not working after CPU cooler upgrade. Other USB peripherals are fine. Help!

After installing a Cooler Master Hyper 212 cpu cooler, a rear exhaust fan and two case fans and, after putting everything back together again, my keyboard (Cooler Master Quickfire Rapid) is unresponsive. The CAPS LOCK and SCREEN LOCK keys are illuminated but it won't recognise any input. Sometimes when I plug it in, Windows 10 says it malfunctioned. Other USB peripherals and even other USB keyboards (tested with a standard Apple keyboard) work fine, just not the CM Quickfire. It does, however, work on other PCs...

I've updated all drivers and done a BIOS update but have the same issue. It's a complete mystery.

[UPDATE 1] Things I've tried which don't work 1) different USB slots.

[UPDATE 2] Running the keyboard with a PS2 adapter *works*. But this is a temporary work around in that it would suggest there's an issue with the USB ports or internal connectors. Any ideas? I have checked all internal connections are fine, which makes me wonder if something is shorting - but that doesn't explain why other USB devices would be fine, does it?

Full specs:
Gigabyte B150M-D3H mobo
Intel Core i5-6500 3.20ghz CPU
Zotac 1060 3GB GPU
8 GB DDR3 RAM
Windows 10

Any thoughts welcome - next step is to dismantle everything, replace the stock cooler, and see if that makes a difference. But before doing that, I was hoping someone else might have a better idea.

Thanks, but this doesn't make a difference - both with the USB ports on the case and the USB ports on the motherboard itself.
