GPU Fans issue

My middle gpu fan randomly stops spinning while the others go full speed, making a lot of noise (as shown in the video), this happens all the time when turning on the PC and during normal usage without even putting load on the gpu.

The graphics card is a GTX 980 Ti (Galax HOF)

Any ideas on how can I fix this?

What PSU is that? I couldn't tell. Looks like the voltages are unstable and making the fans go haywire.

Monitor the temps and voltages in HWmonitor or HWinfo and see what they look like or are fluctuating a lot. Use MSI Afterburner to force the fans to run at 50% constantly and see if the problem persists. If it fixes it, create a custom fan curve in Afterburner.

PSU is XFX XT 600w, its under the cover.

I looked at the GPU voltage in MSI afterburner and everytime the middle fan stops, voltage go up from 875mv to 1037mv, this happens everytime I open any program or just randomly, temperature is around 23-30c. Already tried to set a fixed fan speed, but it still happens.
