GTX 1060 6GB Black Screen While Gaming

Ever since I bought newer computer parts to upgrade my gaming pc I have been having troubles with my GPU. I will be in the middle of a game (probably 15 minutes in) and all of a sudden my screen goes black and my GPU fan will shoot to 100% speed. I have to restart the computer to be able to see my screen again. Does anyone know what is causing this?
My PC specs are:
MSI B150M Mortar LGA 1151
Intel Core i5-6500 Skylake Quad-Core 3.2 GHz LGA 1151 65W
G.SKILL Ripjaws V Series 8GB DDR4 2133
EVGA GeForce GTX 1060 SC GAMING, ACX 2.0
APEVIA ATX-CW500WP4 500W ATX
Windows 10

I checked PCPartPicker before I bought the parts and the website said that everything matched just fine. I also RMA the GPU back to EVGA and when it came back the same thing was happening. I don't know what is going on.

Solution
I don't think it is your GPU, sounds more like overheating issues with your CPU. What are your temps under full load?
Also your PSU is a crappy one, and while 500w should be more than enough to power your system, chances are that your PSU cannot deliver the Juice.

I will run some tests and get you the temps with MSI Afterburner. Do you suggest going ahead and getting another PSU?

I played some PUBG and my CPU and GPU both were hovering around 65 Celsius when my computer black screened
UPDATE: I just bought a new PSU and will update this thread when it comes in.

That is in the fine range, so it seems like it is not overheating. I'm still suspecting that crappy PSU to be the reason.
