Upgrading Graphics Card

Hey, I got a amd radeon r7 250 series it can run Fallout 4 at medium fine, but I'd love it if I could upgrade to a Graphics Card that can take it to high with no lag, or even just getting GTA V to medium, I'm not sure how well GTA will run on my computer though so I haven't gotten/ tried it yet.

Can You Run It said all my other computer specs are good besides my graphics card, although that site is accurate to me like 50% of the time.

I don't really know much anything about Graphics Cards or how to install or replace them and what cards will or won't be compatible with my computer, so advice on that would help too.
I don't really care about price as long as it aint over like $200.

Regarding compatibility, there are really only two issues you need to be aware of: 1), if your power supply has the required PCIe power adapters for whatever video card you choose (one or two 6 or 8 pin PCIe power connectors...always listed in the power supply specs), and 2) if the card you choose will fit inside your case.

You don't give any details on what your case and power supply are, or if it's an OEM like from HP or Dell. So nobody can offer further advice without that.
