It's old fashined postage address.
Obviously, I didn't put in real address/postal code (okay, postal code is for some part of oklahoma city but still)
If you don't know the details, ask your parents, they are all basic information that would be needed if anyone wants to send you something through mail. (like bills for online purchases)
Obviously I cannot fill it out with right information since I don't know your first or last name or where you live.

You also live in somekind of city area and it's in a state (might be left out if you pick something besides US, which might also mean you'd be ineglible for it since a LOT of online rewards specify "must be US citizen"
and you also have ZIP/postal code.
They ask said things supposedly so that single person wouldn't create lots of accounts and/or try to get free "money" aka zsilver.
